    Diş Çekimi Sırasında Apikal Kök Kırıklarının Tanısının Farklı Radyografik Teknikler Kullanılarak Değerlendirilmesi: Ex-vivo Bir Çalışma

    Amaç: Bu çalışma, farklı branşlarda uzman diş hekimleri tarafından yapılan diş çekimlerinde meydana gelen apikal kök kırıklarının teşhisi için farklı görüntüleme tekniklerinin tanı yeteneğini incelemeyi amaçlamaktadır. Yöntem: Diş Hekimliği Fakültesi'nde eğitim amacıyla kullanılan kuru insan alt çeneleri ve rutin tedavi için çekilen dişler kullanılmıştır. Kök uzunlukları periodontal sonda kullanılarak ölçüldükten sonra örnekler farklı uzunluklara ayarlanmıştır. Bu örnekler kuru bir insan alt çenesine yerleştirilmiş ve bir periapikal cihaz, panoramik cihaz ve konik ışınlı bilgisayarlı tomografi (KIBT) kullanılarak görüntüler alınmış ve kaydedilmiştir. Radyografiler ve kayıtlar bir oral ve maksillofasiyal radyolog tarafından alınmıştır. Değerlendirme süreci bir maksillofasiyal radyolog, periodontolog ve oral ve maksillofasiyal cerrah olmak üzere üç kisi tarafından gerçekleştirilmiştir. Bulgular: Periapikal radyograflarda 1 mm kök varlığı/yokluğu konusunda tüm gözlemciler arasında yüksek düzeyde anlamlı uyum olduğu gösterilmiş olup 2 mm ve 3 mm köklerin varlığında, tüm gözlemciler radyografide köklerin görülebildiğini belirtmiştir. Panoramik radyograflarda 1 mm kök varlığı/yokluğu konusunda, tüm gözlemciler arasında orta düzeyde, 2 mm ve 3 mm kök varlığı / yokluğu tanısında ise gözlemciler arasında zayıf bir uyum olduğu gözlenmiştir. KIBT’de ise kök varlığı ve yokluğu tanısında gözlemciler arasında uyum konusunda anlamlı bir farklılık oluşurmuştur. Sonuç: Literatürle uyumlu olarak, bu çalışma KIBT'de gözlemciler arasında daha yüksek uyum olduğunu göstermiştir. Ancak, işlem sırasındaki hastanın kaygısı, lokal anestezinin süresi ve hekimin yorgunluğu göz önüne alındığında, genellikle daha uzun bir görüntü işleme süresine sahip olan KIBT yerine genellikle iki boyutlu radyografiler tercih edilmektedir. Periapikal radyografiler kullanılarak yapılan tanıların, panoramik radyografiler kullanılarak yapılan tanılardan daha etkili olduğu görülmüştür.

    Evaluation of cone beam computed tomography videos on Youtube

    Purpose: Cone beam computed tomography (CBCT) is an imaging modality that plays an important role in dental practice. However, it is unclear whether videos with adequate and accurate information are available on a platform such as YouTube. The aim of this study was to determine whether CBCT videos posted on YouTube are indeed a valuable source of information. Methods: During the data collection process, we analyzed Google Trends searches using the keywords "cone beam computed tomography (in Turkish)", "dental tomography (in Turkish)", and "tooh tomography (in Turkish)" over the last 5 years. Then, 168 videos were analyzed as a result of searches on YouTube with these keywords. Among these videos, 48 were included in the study according to certain criteria. The number of view, viewing times, number of like, interaction index and viewing rate of the included videos were calculated. The upload source of the videos was categorized as clinic, dentist, educational channel, commercial, professional organization and TV channel. According to the upload source of the videos, the image quality, sound quality, narrator, purpose of the video, usefulness scoring and Global Quality Scale were evaluated. Results: The average number of views of the analyzed videos was determined as 1559.95, while the standard deviation value was determined as 2435.30. The average interaction index was 1.58, and the standard deviation value was 1.77. The average view rate was 377.55, and the standard deviation value was 956.88. Significant relationships were found between the upload source of the videos and the duration of the video, the number of subscribers of the channel that uploaded the video, and the number of likes (p=0.016, p=0.021, p=0.030, respectively). In addition, when the videos were evaluated in terms of their usability levels, significant relationships were found between the duration of the video, the number of likes, and the interaction index (p=0.001, p=0.009, p=0.015, respectively). Conclusion: This study reveals that CBCT videos posted on YouTube are generally of low quality and not informative. This highlights the need for reliable sources of information for dentists, patients, and future dentists.

    INVESTIGATION OF CONDYLE POSITIONS IN PATIENTS WITH TEMPOROMANDIBULAR JOINT DISORDER BY CONE-BEAM COMPUTED TOMOGRAPHY

    Objective: The condylar position refers to the location of the mandibular condyle within the glenoid fossa when the teeth are in maximum intercuspal, and it is an important indicator of the temporomandibular joint (TMJ) function. This study aimed to assess the condylar position of the TMJ in patients with TMJ disorder using cone-beam computed tomography (CBCT). Methods: The present study utilized CBCT to evaluate the condylar position of the TMJ in a sample of 56 patients. Among them, 28 patients were asymptomatic while the remaining 28 had been diagnosed with TMJ disease. To evaluate the presence of TMJ disorders, the Research Diagnostic Criteria for Temporomandibular Disorders was employed. This diagnostic tool is widely used in clinical and research settings to assess TMJ-related pain and dysfunction. Results: The agreement was excellent, according to the intra-observer and inter-observer correlation values obtained. There was no statistically significant difference between the two groups in terms of age and gender distribution. The results showed that the anterior and posterior joint spaces were significantly higher in the symptomatic group compared to the asymptomatic group. However, there was no statistically significant difference between the two groups in terms of superior joint space and articular eminence inclination values. Gender did not create a significant difference in any parameter. Conclusion: The results of this study emphasize that CBCT images have an important role in detecting significant changes in condyle position in patients with TMJ disorders. However, further large-scale studies are necessary to validate and enhance the findings.

    Prevalence of persistent apical periodontitis in patients with root canal-filled teeth: A cone-beam computed tomography study

    Aim: Apical periodontitis is the inflammatory destruction of periradicular tissues that occurs gradually over time, primarily originating from invading bacteria in the root canal system and initiated by the release of bacterial products. In this study, the prevalence of post-treatment apical periodontitis was assessed with cone-beam computed tomography imaging and the effects of tooth-level and patient-level factors that may predict the failure of root canal treatment were investigated in a Turkish subpopulation.Material and Methods: Our study included 305 cone-beam computed tomography images of patients (153 males, 152 females aged 18 to 78 years) [Mean & PLUSMN; standard deviation (SD) = 39.2 & PLUSMN; 17.6].Results: Post-treatment apical periodontitis prevalence was 83.3% and was more common in the premolars (9.5%), the mandible (11.8%), and the left side of the jaw (11.8%).Discussion: This study showed a high prevalence of post-treatment apical periodontitis in a Turkish subpopulation even in cases in which the quality of root canal filling appeared to be sufficient radiologically. Cone-beam computed tomography is one of the most sensitive imaging techniques for the detection of apical periodontitis.

    SİGARA KULLANIMININ KANAL TEDAVİSİ SONRASI PERİAPİKAL İYİLEŞME ÜZERİNE ETKISİNİN İNCELENMESİ

    Amaç: Bu çalışmanın amacı, sistemik açıdan sağlıklı ve sigara kullanan bireylere karşı sistemik açıdan sağlıklı ve sigara kullanmayan bireylerin kök kanal tedavisi (KKT) sonrası periapikal lezyon alanlarındaki kemik trabekülasyonunun kantitatif değişimini fraktal analiz ile karşılaştırmaktı. Gereç ve Yöntem: Bu çalışmanın verileri, deney grubunda sigara kullanan ve herhangi bir sistemik hastalığı olmayan 17 adet hastaya ait 17 adet periapikal lezyonlu mandibular diş (n=17) ve kontrol grubunda sigara kullanmayan ve herhangi bir sistemik hastalığı olmayan 19 adet hastaya ait 19 adet periapikal lezyonlu mandibular diş (n=19) olmak üzere 2 grupta toplam 36 dişten elde edildi (N=36). Kanal tedavisinden önce ve tedaviden 1 yıl sonraki radyografiler değerlendirildi. Radyografilerde ilgilenilen periapikal bölge seçilerek fraktal analiz yapıldı. Fraktal boyut değişiklikleri hesaplanarak periapikal iyileşme değerlendirildi. Verilerin her iki grupta normal dağıldığı tespit edildiğinden, sigara kullanan ve sigara kullanmayan gruplar arasındaki periapikal iyileşme miktarlarının karşılaştırılmasında bağımsız t testi kullanıldı. Grupların kendi içerisinde KKT öncesi ve KKT’den 1 yıl sonraki fraktal boyut değerleri bağımlı t testi ile karşılaştırıldı. P ? 0.05 değeri istatiksel açıdan anlamlı fark olarak kabul edildi. Bulgular: Hem sigara kullanmayan (p=0.000) hem de sigara kullanan (p=0.000) grupta KKT’den 1 yıl sonra istatistiksel açıdan anlamlı derecede önemli fraktal boyut değeri artışı tespit edildi. Sigara kullanan ve sigara kullanmayan gruplar arasında fraktal boyut artışı açısından istatistiksel açıdan anlamlı fark bulunmadı (p=0.774). Sonuç: Kanal tedavisinden 1 yıl sonra her iki grupta da istatistiksel açıdan anlamlı derecede önemli periapikal iyileşme tespit edildi. Sigara kullanımının KKT sonrası fraktal boyut artışı üzerinde istatistiksel açıdan anlamlı derecede önemli etkisi tespit edilmedi. Anahtar Kelimeler: Fraktal boyut, Periapikal iyileşme, Apikal periodontitis, Sigara kullanımı,Aim: The aim of this study was to compare the quantitative change of bone trabeculation in periapical lesion areas after root canal treatment (RCT) with fractal analysis in systemically healthy smokers versus systemically healthy non-smokers. Materials and Methods: The data of this study were obtained from a total of 36 teeth in 2 groups (N=36). In the experimental group, data were collected from 17 mandibular teeth with periapical lesions (n=17) belonging to 17 patients who were smokers and did not have any systemic disease, and in the control group, data were extracted from 19 mandibular teeth with periapical lesions belonging to 19 patients who did not smoke and did not have any systemic disease (n=19). Initial radiographs and 1 year after root canal treatment were evaluated. Fractal analysis was performed by selecting the region of interest in radiographs. The changes of fractal dimensions in the lesion area were calculated and compared for both groups. Since the data were found to be normally distributed in both groups, the independent t-test was used to compare the periapical healing between the smoking and non-smoking groups. For within group comparisons, the fractal dimension values of before RCT and 1 year after RCT were compared with the dependent t-test. P ? 0.05 was set as the limit of significance. Results: There were statistically significant increase in fractal dimension values of 1 year after RCT in both non-smoker (p=0.000) and smoker (p=0.000) groups. There was no statistically significant difference in fractal dimension increase between smoker and non-smoker groups (p=0.774). Conclusion: A statistically significant periapical healing was detected in both groups 1 year after root canal treatment. No statistically significant effect of smoking was observed on fractal dimension increase after RCT. Keywords: Fractal dimension, Periapical healing, Apical periodontitis, Smoking

    The effect of temporomandibular joint dysfunction on the craniocervical mandibular system: A retrospective study

    Background: Temporomandibular disorders are the most common condition affecting the orofacial region, resulting in pain and dysfunction.Objective: This study aimed to elucidate the ambiguous association between cervical features and temporomandibular disorders by measuring the rotations between the skull-atlas, atlas-axis and mandible-atlas and examining the relationship between these rotations and temporomandibular disorders.Methods: Cone-beam computed tomography (CBCT) images from 176 patients, 97 females and 79 males with an average age of 25.7 years were used in this study. The patients were divided into two groups: those with joint dysfunction (n = 88) and those without (n = 88). The study employed various methods to determine rotations in the skull-atlas, atlas-axis and mandible atlas based on anatomical landmarks and measurements. These methods include the use of specific planes, angles and distances to identify and measure rotation. Data analysis was performed using the TURCOSA statistical software (Turcosa Analytics Ltd Co, Turkey, www.turcosa.com.tr).Results: The results showed that the degree of rotation between the skull and the atlas was higher in the TMD group than in the control group (p < .001). Similarly, Atlas-axis rotation was significantly higher in the TMD group (p < .001). However, no significant difference was found between mandible atlas rotations in the two groups (p = .546). The study also found a significant difference between the direction of rotation between the atlas and axis and the direction of mandible atlas rotation (p < .001) as well as between skull and atlas rotations and mandible-atlas rotations (p < .001).Conclusion: Overall, the study suggests that there is a relationship between the skeletal structures of the cranio-cervico-mandibular system and TMD. Skull-atlas and atlas-axis rotations may play an important role in the aetiology of TMD in individuals with TMD. Therefore, it is important to evaluate rotations in the skull-atlas-axis region for the treatment of TMD.

    The Relationship of Fractal Dimension and Osseointegration: A Retrospective Radiologic Clinical Trial

    Objective: The objective of this study was to evaluate the density of alveolar bone in the peri-implant area using fractal analysis on orthopantomograms, both prior to and following clinical osseointegration. Material and Methods: Orthopantomograms were performed prior to the implant surgery and again four months postoperatively. Regions of interest at the mesial, distal, and apical sites of the implants were identified for analysis. The method by White and Rudolph was employed to calculate the fractal dimension. Subsequently, fractal dimension values were compared across all recorded measurements. Results: Statistical analyses revealed significant correlations between the mesial measurements before and after surgery, as well as between the distal measurements pre- and post-operatively, compared to the apical measurements over the same periods. Further subgroup analyses identified significant correlations in pre- and post-operative fractal dimension values specifically within the female subgroup, with notable distinctions observed between pre-mesial, post-mesial, and the average values before and after surgery (p
